Drawing on experience from autonomous vehicles, advanced robotics and more than a decade of cybersecurity incident response, this paper introduces a two-part model: the AI Risk Hierarchy, which ranks the reputational severity of where and how AI is deployed, and the AI Accountability Requirements, the criteria organizations need in place to communicate effectively when an AI incident happens.
